That’s why we’ve created our In-Office Membership Plan—a simple, low-cost monthly plan designed to meet the healthcare needs of our uninsured patients.
 

Registration Fee: $50.00 per person Monthly Membership Fee: $40 per person

• ✓  Annual Exam and three (3) main blood work (CBC, CMP, LP) & EKG if needed

• ✓  Members can schedule unlimited in-person or Televisit visits at our Coral

  Springs location for primary care services.

• ✓  EKGs

• ✓  Injections

• ✓  Minor in-office procedures

  Cancellation Policy:
   

  Members who choose to cancel their plan within the first six (6) months of enrollment will be subject to a $120 early cancellation fee.
